The "Vaccination Station" at Humane Society of West Michigan

 
The "Vaccination Station" is a partnership between Humane Society of West Michigan and C-SNIP in an effort to offer affordable options to low income pet owners. 
 
 
WHERE: Humane Society of West Michigan (3077 Wilson NW, Grand Rapids, MI 49534)
WHEN:  2nd Monday of the month from 2pm-5pm, walk-in only - no appointments *We reserve the right to limit entrance 30 minutes before closing based on the number of people waiting.

YOU MUST LEAVE YOUR PET(S) IN THE CAR UNTIL AFTER

YOU ARE REGISTERED AND ASKED TO BRING THEM IN

ALL PETS SHOULD BE ON LEASH OR IN A CARRIER

Documentation of low income status is required.  Verification must be one of the following:

Bridge card - and annual letter dated within the last 12 months & card, in the name of the pet owner

Medicaid - letter/statement and copy of card, in the name of the pet owner.                                                                                                                        

SSI Disability / Retirement - letter/statement, in the name of the pet owner.

Unemployment / Salary statement - Must be the name of the pet owner. (Less than 200% of Federal Poverty Level guidelines

WHERE: Cook (1100 Grandville Ave SW, Grand Rapids, MI 49503)

WHEN: June 21st from 2-5PM  (We reserve the right to close doors at 4:30)

MUST BRING PROOF OF LOW-INCOME

 

Alternative Vaccine Clinic

WHERE: C-SNIP (1675 Viewpond Dr. SE, Kentwood, MI 49508) 
WHEN:  3rd Friday of the month from 11am-2pm, walk-in only - no appointments
