How do I qualify for the community vaccine clinic?
To participate in our clinic please bring the following paperwork with you:
Photo ID of the pet owner and participant in a low-income assistance program
Pet medical history documents showing your pet's past vaccines and proof of spay/neuter
Current low-income status documents proven by one of the following:
MDHHS Food Assistance program - annual letter of qualification dated within the last 12 months.
Medicaid or Medical Assistance letter of qualification
Social Security Disability or Retirement letter of qualification
2 recent pay stubs, for all working members of the household, showing less than 200% Federal Poverty Level (Proof of income for ALL residents of the household required. Less than 200% is based on all residents living in the home. Details can be found online by searching: "federal poverty guidelines 2021")
*All animals in the household must have proof of being spayed/neutered to receive assistance.*
What if my pet is not spayed or neutered?
Any animal that has not been spayed (female) or neutered (male) prior to visiting the HSWM Vaccine Clinic, must purchase a $10 Spay/Neuter Voucher. When the voucher is presented the day of surgery at HSWM, you will receive $10 off the cost of the surgery.
At this time, Spay/Neuter services are only available for CURRENT Vaccine Clinic clients.
